No tape measure needed — a bank card is the ruler. Get these
three things right and the measurements come out clean.

Smooth it flat — don't stretch it. Pat the
wrinkles out, but let the fabric relax; pulling it taut adds width. Sleeves
down beside the body, not out to the sides.

Tips for a good scan
Use a contrasting surface — a grey top on grey carpet can't be measured. A bed sheet or towel in a different colour works well.
Boxy tees — if the sleeves won't sit down by the sides, fold them back over the front so the outline is just the body.
Stretchy fabric — a soft jersey tee can read a couple of cm wider or narrower depending on how much it's pulled. Lay it the same relaxed way each time, and don't stretch it flat.
Trousers and jeans — lay them out normally, legs apart. No folding needed.
